Hand print your name, complete address, daytime phone number, birth date and email address on a 3" x 5" card and mail in a first class stamped envelope to: 2007 Sears Survey Sweepstakes, PO Box Number 5040, Ellenton, FL 34222-5000. Mail-in entries must be postmarked by January 31, 2008 and received by February 7, 2008. Each mail-in entry must be mailed separately. No copies, facsimiles or mechanical reproductions will be accepted.
