I'm amazed that so many people pay for food they can't eat.
If the customers refused to eat from the dirty tables I'm sure they'd clean up their act pretty quickly.
I liked your letter. It covered all the areas I can imagine wanting to know about.

Eww..the smell of bug spray makes me sick let alone eating along with it! You should have called the headquarters and complained that the spraying is unacceptable when there are people in the vicinity eating. Futhermore it seems like they have a pest problem

That is absolutely disgusting.
It is clear from the health-reports that this location is a loss with no interest in cleaning up their operation.
I am encouraged that OP is deciding to no longer visit this establishment, though if it had been my case, I would have either left after the spray was distributed, or I would have denied accepting the food and walked out of the building, posthaste.
Blech. I hear nice place's kitchens are bad enough. What is THIS IHOP's kitchen like?

and had a lovely conversation with the server about the color of her underwear. She told me I guessed the right color, but I didn't obtain further confirmation.
I can't remember anything about the food on that visit. I used to order the international crepes, but the fact of the matter is they were kind of gross.

Look at the list of inspections and results Bill posted below.
Here is one snippet:
Critical Procedures Inspection 21-Sep-2007 4 critical
Follow-up Inspection 8-Feb-2007 2 critical
Routine Inspection 29-Jan-2007 6 critical
If the health department doesn't see fit to do more inspections after 3 consecutive visits with multiple critical violations, one can see how some uncaring managers might conclude that keeping a clean environment doesn't matter.

dirty menu... now that one i can overlook, especially if i'm in a good mood.
i had that happen at an ok-ish restaurant just last week. asked for a clean one and was given a clean menu and they were polite (actually they looked embarrassed) but... eh they fixed it so no worries.
as for the dust and hair on the stuff... yum yum.
i can understand not wanting to go back.
if they will not respond to any of your comments there is one thing you can do that will get a response...
call the board of health. they can't ignore the boh when they come knocking to inspect the place. =)

Most likely the spray was a cleaner. If they mix their own it might smell very strong if they dont add enough water.
As for everything else, I would have walked out after seeing hair on the syrup bottles.
If they wont take your phone calls just give up, its obvious they dont take complaints very well. Just show them your displeasure by not going again and telling other people about it.
