Unable to get approved Intel rebate sent out

Can not get approved $50 Visa rebate sent out for Intel 520 180gb SSD purchased in May 2012. Have contacted supplied phone number 888-401-2627 to no avail for multiple times. Always say supervisor will call back, but never have gotten a call back nor resolution to issue.
